2008 W-4   Employee election form for Federal tax filing status for payroll withholding.  Required of each new employee and can be changed by the employee at any time by completing a new W-4 form.

DE-4   Election form for State tax filing status for payroll withholding.  Only required if an employee wishes different tax filing status for state than for federal.  The W-4 election can cover both if the tax filing status is the same for state.

EE Direct Deposit Agreement   Employee election form for direct deposit of payroll check.  The election can be changed by submitting a new form.  Follow directions on the form - important to enclose a voided check for checking accounts and the bank provided routing and transit numbers along with account number for savings accounts.

I-9 Form   Employment Eligibility Verification - required to be completed by employers on each employee within 3 business days of the date their employment begins to verify and document the employee's eligibility to work in the United States. 2008 revision
